Scheduling Request Form
Please complete the form below in order to request use of the Town Hall Gymnasium. You must complete your scheduling request a minimum of 5 days before the requested date. Please read the Gymnasium Policies & Procedures prior to completing the form. You will receive an email acknowledging your request and a second informing you whether your request is approved or denied along with further instructions on gaining access to the facility.
